Generic Drugs Medications are listed both by brand name and generic. Brand (trade) names are listed in bold text and generic (chemical) names are listed in italics. The classification of a prescription drug product as a generic is determined by us based on commercially available data resources. When generic substitution conflicts with state regulations or restrictions, the pharmacist must obtain approval from the prescribing physician to substitute the generic equivalent. Physicians who think that a brand name drug is required when an approved generic version is available should contact the Pharmacy Call Center ( 1-800-310-6826) to provide the clinical information that supports the request.
