AmeriChoice to Offer Care Management Services to the Florida Medicare Population

AmeriChoice Management Services Organization (MSO) has been selected by LifeMasters Supported SelfCare, Inc. (LifeMasters) to provide case management services to a group of medically fragile and high-risk residents of Broward, Dade and West Palm Beach counties in Florida who are eligible for both Medicare and Medicaid.

AmeriChoice MSO is a business segment of AmeriChoice that focuses on health management programs for Medicaid, dual eligible and uninsured populations across the country. The AmeriChoice MSO care program model, rooted in evidenced-based, best practice guidelines, features a patient-centric, physician-driven philosophy that promotes optimal health and well being for the patient through both telephonic and face-to-face interactions with individuals and their physicians.

For this new program, AmeriChoice MSO will provide field-based case management and support services for LifeMasters, a leader in the disease management field. AmeriChoice’s responsibilities will include: coordinating multiple case management programs, integrating and managing patient information, completing environmental and needs assessments, integrating community programs, coordinating and assessing the appropriateness of specific services, and educating patients and their families about health topics.
