AmeriChoice, a UnitedHealth
Group company that is solely dedicated to public sector health programs,
has been selected by the Georgia Department of Community Health (DCH)
to provide disease management, case management, and wellness services
to approximately 50,000 Medicaid eligible aged, blind, and disabled
individuals in 106 Counties across the Central and Southern regions
of Georgia.
The AmeriChoice Management Services Organization (MSO),
a business segment of AmeriChoice that focuses on health management
programs within fee-for-service Medicaid and uninsured program
environments, will administer this program. AmeriChoice MSO blends
administrative and medical management services to create health care
programs that promote both optimal health outcomes and cost-savings.
The AmeriChoice MSO care program model, rooted in evidenced-based
best practice guidelines, boasts a patient-centric, physician driven
philosophy that promotes optimal health and well being for the patient
through both telephonic and face to face interactions with Medicaid
recipients and their physicians. AmeriChoice MSO will also provide
physician support services, twenty-four hour nurse triage services,
patient and provider profiling, and clinical outcome measurement
services as part of this program.
