Since 2003, AmeriChoice has participated as a program partner in the Indiana Chronic Disease Management Program. In collaboration with the Indiana State Department of Health, the Regenstrief Institute and the Indiana Primary Healthcare Association, AmeriChoice participates in managing the State-administered program. AmeriChoice provides telephonic disease-specific education and self-management skills training to approximately 20,000 individuals statewide.
