AmeriChoice’s strategy for outreach is achieved through building strong relationships with clients, providers, community-based organizations, and other stakeholders, through consistent and open communication. Our care managers and administrative staff work diligently to create and strengthen relationships with community resource agencies capable of supporting this program and assisting clients in achieving their health status goals.

Role of a Community Partner
To maximize client contact and engagement for hard to reach clients, we call upon our community partners. Agencies may be selected for inclusion in the Community Partner Network (CPN) based on their core services and geographical coverage. The CPN allows for increased program support from the communities, clients, and providers. With the help of CPNs we have seen improved program outcomes in the following areas:

• Locating hard to reach clients,
• Increasing provider buy-in of program,
• Improving receptivity of program and program staff in local communities,
• Improving satisfaction with the program among clients, providers, and communities, and
• Helping program keep abreast of community and client changes as they evolve.
