AmeriChoice welcomes you to the CCM Program! This care management program is sponsored by the Washington Department of Social and Health Services and is designed to:

• Identify clients who need care management
• Support medical homes for clients
• Improve the health of clients by promoting evidence-based medicine
• Work with clients to prevent avoidable medical costs

AmeriChoice is skilled in helping people with chronic diseases. Our goal is to help clients understand how to better manage their conditions. A major focus is placed on increasing clients’ knowledge about their illnesses, including potential complications, the importance of medication, and treatment plan compliance. We employ both nurses and social workers to directly interact with clients, their families/caregivers, and doctors.
